ORGANIZATIONAL RELATIONSHIP:  Care Manager

JOB SUMMARY

An individual who is competent to perform assigned functions of non-medical care to the client in their residence.

QUALIFICATIONS

Prefer Completion of the 9th grade; prefer high school graduate or equivalent

Ability to read and write in English, consistent with job requirements

A reliable means of transportation to get to assigned patient visits

Must have a criminal background check and current CPR certification

Meets one of the following:

§  An individual who is competent to perform assigned functions of non-medical care to the client in their residence.

RESPONSIBILITIES/ESSENTIAL FUNCTIONS

Tasks to be performed by a PCA must be assigned by and performed under the supervision of the Agency Care Manager who will be responsible for the patient care provided by the Agency

Duties of the Personal Care Assistant may include:

Activities of daily living (bathing, grooming, dressing)

Assisting with meal prep, grocery shopping, meal planning

Range of motion/exercises,

Transfers / use of mechanical lifting devices

Vital sign checks: temperature, pulse, respiration

Home Management Tasks: laundry, meal preparation, bed making etc.

Assisting with ambulation, transfers, turning and repositioning

Accompany program participants to activities of their choice

Able to access patient’s homes that may not be handicap accessible.

Able to hear, speak, understand and communicate effectively in English.

PHYSICAL REQUIREMENTS

On a daily basis: walking, standing, sitting, reaching, use of telephone, use of computers & other office equipment, bending, stooping, climbing stairs.

Provide shopping service to client to include grocery and clothes shopping.
Provide incidental transportation for client which includes visits to doctor appointments,
friends, family members, or stores.
Provide assistance and planning of client’s recreational activities and events.
May remind client of medications.

WORK ENVIRONMENT

Indoors and outdoor providing companionship to patient in location of choice.

Travel to patient residences/meetings; will require transportation to get to various work sites

Possible exposure to communicable diseases

Housekeeper to manage residence in conjunction with Employer/Client.

Assist client as needed on all and any daily tasks.

This position requires someone to be proactive, anticipatory, well organized, highly attentive to detail, flexible and willing to be hands-on an on-going basis. A self-starter with good steady energy.

Involved Tasks
This role includes a variety of tasks, light cleaning, laundry, occasional project management, daily organization, errand running and shopping, cooking and serving, driving. Most importantly, this position is for someone who is trustworthy and committed to support the Employer.

Requirements

  • Similar employment experience with verifiable references
  • Willing to undergo full background checks and signs a confidentiality agreement
  • Computer proficient
  • Clean and valid driver’s license
  • Physically be able to perform all tasks required for position, lift up to 35 pounds and climb ladders as appropriate (e.g. change lightbulb)
  • Strong work ethic
  • Non-smoker
  • Flexible in schedule, generally full-time, Monday-Friday  and/or weekend schedule
  • Strong command of the English language. Bilingual a plus.

Job Summary:

An OT provides professional services within the scope of occupational therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families. OT in the home is performed under the supervision of the RN case manager, Administrator, DCS. If Nursing has opened the case, Occupational Therapy is a secondary discipline and therefore works collaboratively and professionally with the RN case manager to provide necessary services.

Qualifications:

  • Graduated after successful completion of an OT education Program accredited by the Accreditation Council for OT Education (ACOTE) of the American OT Association (AOTA) or successor organizations and is eligible to take or has successfully completed the entry level certification exam for OTs from the NBCOT
  • A currently licensed in MA
  • Will have a minimum of two (2) year experience as a OT, preferably in a homecare setting
  • Criminal check & Current CPR certification
  • If educated outside of the United States must meet both of the following:
  1. Graduated after successful completion of an OT education program accredited as substantially equivalent to OT entry level education in the US by one of the following: Accreditation Council for OT Education (ACOTE), successor organizations of ACOTE, the World Federation of OTs, a credentialing body approved by the American OT Association, successfully completed the entry level certification exam for OTs developed & administered by the National Board for Certification in OCC Therapy (NBCOT)
  2. On or before December 31, 2009, is licensed or otherwise regulated, if applicable, as an OT by the state in which practicing

Job Summary:

A Physical Therapist provides professional services within the scope of physical therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families.

Qualifications:

  • Successful completion & graduate of a PT Program approved by the Commission on Accreditation in PT education (CAPTE) or successor organizations of the CAPTE or an education program outside of the United States determined to be substantially equivalent to physical therapist entry level education in the US by a credentials eval organization approved by the American PT Association or an organization identified in 8 CFR 212.15(e) as it relates to physical therapists
  • A current, active license to practice Physical Therapy in MA
  • A minimum of two (2) years’ experience as a Physical Therapist
  • Criminal background check & Current CPR certification
  • If trained outside of the United States before January 1, 2008, meets the following requirements:
  1. Was graduated since 1928 from a PT curriculum approved in the country in which the curriculum was located and in which there is a member organization of the World Conference for Physical Therapy
  2. Meets the requirements for membership in a membership organization of the World Confederation for Physical Therapy

Job Summary:

A professional who provides professional nursing services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates patients and their families. Professional skilled nursing is performed under the supervision of the Administrator/Director of Clinical Services.

Qualifications:

  • Graduate of an approved professional nursing program (RN)
  • Currently, licensed “in good standing” in MA
  • Minimum of one (1) year of experience as a professional nurse, preferably in home care
  • Must have a criminal background check
  • Must have current CPR certification
